Whole-Person Growth at Thrive at Life

Welcome to Thrive at Life!

Thrive at Life is a community-based, group day program that offers one-on-one Life Skills Coaching for each participant.


While participants engage in group activities and shared experiences, each individual is paired with a dedicated Life Skills Coach who provides personalized guidance, support, and encouragement throughout the day. This allows us to tailor support to each participant’s goals, strengths, and needs while still fostering community, connection, and independence.

One-on-one coaching focuses on:


  • Daily living and independence skills
     
  • Social communication and participation
     
  • Emotional regulation and coping strategies
     
  • Responsibility, routines, and work readiness
     
  • Building confidence and self-advocacy
     

Thrive at Life is not a therapy program, school, or medical service. Life Skills Coaching is practical, strengths-based, and focused on everyday growth.


What the Intake Includes



Our intake form asks questions about:

  • Daily living skills and independence
     
  • Communication and social support needs
     
  • Strengths, interests, and goals
     
  • Sensory, regulation, and support considerations
     
  • What the individual and family are hoping to gain from the program
     

You are encouraged to share only what you are comfortable with. There are no “right” or “wrong” answers.


Who Completes the Intake Form


  • Participants age 18 and older complete the Adult Participant Intake Form, unless there is documented legal guardianship.
     
  • Parents or legal guardians complete the Parent/Guardian Intake Form for minors under 18 or for adults with court-appointed guardianship.
     

If you are unsure which form to complete, we’re happy to help guide you.


What Happens After You Submit the Intake


After we receive your intake form:


  1. Our team reviews the information to assess program fit.
     
  2. We may reach out to schedule a call to learn more.
     
  3. Families will be notified of next steps, which may include enrollment, a waitlist, or referrals to other resources if Thrive at Life is not the right fit at this time.
     

We aim to be thoughtful, transparent, and respectful throughout this process.


A Note on Our Program


Thrive at Life is a community-based life skills and enrichment program.


We are not a therapy provider, school, or medical service, and we do not offer crisis or one-on-one therapeutic care.


Our focus is on whole-person growth — emotionally, socially, physically, and spiritually — within a supportive group environment.
